<p>Firstly, there could be a definite cause found for sterility, where no time spends to enable pregnancy. Sterility can be due to no sperm cells in the ejaculate. It can result from a complete absence of ovulation; worst case scenario there may be no eggs in the ovaries. It can result from a complete obstruction to the fallopian tubes, which prevents sperm from reaching an ovulated egg to fertilize it. Or the embryos that result from fertilization can repeatedly fail to develop, perhaps because the eggs are abnormal or, less commonly, because disease of the uterus ruins implantation.</p>
<p>Secondly, cause may be due to sub-fertility, by which we mean fertility that is low in production to what we expect, but is not zero. Examples include a low sperm count; unstable ovulation; or problem of anatomy or physiology that lowers the chance of sperm or egg arriving for fertilization at the right time at the right place, like an immune reaction against sperm (sperm antibodies); or an abnormality in the uterus disturbing implantation.</p>
<p>Thirdly, there is nothing wrong whatsoever. Unfortunately, the infertility is unexplained.</p>
<p>The temptation with unexplained infertility is to discover new causes of infertility or, failing this, to think of the infertility as psychological. As it turns out, the only major new cause of infertility that has been discovered in the last 15 years is the harmful effect aging has on a woman&#8217;s eggs.</p>
<p>There is new hope, IVF technology has given us new insights into what can sometimes go wrong, but it is still uncommon to discover a barrier to getting pregnant with IVF. Generally, for women under 35 having IVF for unexplained infertility often uncover a previously unsuspected sperm problem in their partner; for women over 35 it more often turns out to be egg-related. However, biologically speaking, many infertility cases are not strictly abnormal.</p>
